What is an NDIS plan review?
Your plan review is the moment to say what's working, what isn't, and what you need next. Preparing properly makes a big difference, and you don't have to do it alone. We help you gather the right evidence, put your goals into words, and walk into the meeting feeling ready.
An NDIS plan review is a meeting, usually held toward the end of your plan period, where the NDIA or a Local Area Coordinator looks at how your plan has gone and decides what your next plan should include. Most plans run for 12 months, but you can also ask for a review sooner if your circumstances have changed.
A good review isn't a test. It's a chance to show how your supports have helped and where there are gaps.
How we can help you prepare?
- Look back over your plan, goal by goal: what you've achieved, what's been hard, and what's changed.
- Gather your evidence, such as reports or letters from therapists, your GP or support workers, and our own support notes.
- Spot gaps and unused funding, and understand why a category was under- or over-used
- Put your goals into your own words, so they describe your real life and not just formal wording
- Plan what to say and bring, including who you'd like with you at the meeting
- Be there on the day, if you'd like us to attend and help you feel confident (the choices and decisions stay yours)

- Notes on what's worked and what hasn't (even a few dated notes help)
- Reports or letters from therapists, your GP or other professionals
- A list of your current providers and how it's going with each
- Any changes in your health, home life, work, study or support network

Frequently asked questions
When should I start preparing for my plan review?
As early as you can. Starting a few months ahead, with notes and reports, gives you much more to work with than starting the week before. If your review has already been scheduled, call us. We'll work with the time you have.
Can I ask for a plan review before my plan ends?
Yes. If your circumstances have changed significantly, you can ask for a review sooner. We can help you explain what's changed and what evidence to include.
Do I need a support coordinator to prepare for a plan review?
No, but it helps. If support coordination is funded in your plan, preparing for your review is part of the role. If it isn't, we can talk with you about the best way forward.
Will you come to my plan review meeting?
If you'd like us there, we can attend to support you. You stay in charge of what's said and decided.
Can support workers help with community access, not just tasks at home?
Yes — community access, building social connections and working toward independence goals are a core part of what our support workers do, alongside daily living support.
How quickly can I start with a support worker?
This depends on your location and needs, but our team will talk you through realistic timing as soon as you get in touch — we'd rather set the right expectations than rush a match that isn't ready.
