Support Coordination
This involves assisting individuals with disabilities or complex needs to navigate and access support services. It involves coordingating various services, resources, and providers to ensure a person receives the right support for them, tailored to their specific needs. This process often involves collaboration with healthcare professionals, community organisations, and support service providers to enhance the overall well-being of the individual. Level 1 Support ConnectionSupport Connection assists you to connect with informal, community and funded supports to help you to get the most out of your plan and achieve your personal goals.
Level 2 Support Coordination – Coordination of SupportsSupport coordination will assist you the participant to build the skills you need to understand, implement, and use your plan. A support coordinator will work with you to choose providers and supports to increase your capacity to meet your personal goals, maintain relationships, manage service delivery, live more independently, and be included in your community.
Level 3 Specialist Support CoordinationSpecialist support Coordination is a higher level of support coordination. This level of support is for people whose situations are more complex and who need specialist support. Our Specialist Support Coordinator team will assist you to manage challenges in your support environment and ensure consistent delivery of service. Specialist support coordinators will assist you the participant to develop your skills to understand, implement, and use your plan. They will work with you to choose providers and supports to increase your capacity to meet your personal goals, maintain relationships, manage service delivery, live more independently and be included in your community.
NDIS plan reviews - why?
Plan reviews are periodic assessments conducted to evaluate the effectiveness of an individuals current support plan. These reviews are crucial to ensure that the plan continues to meet the evolving needs and goals of the person with a disability.
How do we help?
The participant (you), along with their support network and sometimes a planner or coordinator (us), discusses the progress made, any changes in circumstances, and whether the existing support networks are adequate. The review aims to determine if any adjustments or modifications are necessary to better address the participant's requirements.
Allied Health Services
We now have a Speech Patholigist available with Amaze Abilities and they are mobile so may be able to come to your home. Our speech therapy program is designed to support individuals with communication challenges, helping them build essential skills to enhance their quality of life.Whether it's improving speech, language, social communication, introducing Augmentative and Alternative Communication (AAC) or feeding and swallowing support, our experienced therapist can assist every step of the way.
